When we talk about dementia risk, weight often gets the spotlight. But emerging research is shifting attention toward something that is easier to overlook: muscle.
A growing body of evidence suggests that body composition, the amount of muscle, fat, and other tissues in the body, may provide information that body weight or BMI (body mass index) alone cannot capture.
This matters because two people can have the same weight or BMI but have very different amounts of muscle and body fat.
Why Might Muscle Matter for the Brain?
Muscle is not simply about strength or appearance. It is metabolically active tissue that plays an important role in blood sugar regulation, movement, and overall physical function.
You can also lose muscle with aging and being physically inactive. Previous research has linked lower lean mass and muscle strength to worse cognitive health, and more recent studies continue to investigate possible links between body composition and neurodegenerative diseases.
For example, earlier research found that people with early Alzheimer’s disease had lower lean mass, and that reductions in lean mass were associated with brain volume and cognitive changes.
Does This Mean Being Overweight Is Not a Dementia Risk?
Not at all.
Excess body fat, particularly around the abdomen, has been associated with several health problems that can also affect brain health. Recent research has also explored biological pathways through which obesity may contribute to Alzheimer’s disease.
The important message is that weight alone may be an incomplete measure of health.
Instead of asking only, “How much do I weigh?” it may also be worth asking: “How much muscle do I have, and how strong and physically active am I?”
The Muscle-Building Habit Worth Paying Attention To
Resistance exercise, such as strength training, bodyweight exercises, or working with resistance bands, can help maintain or build muscle.
And you do not necessarily need to spend hours in the gym.
A 2026 study reported that 90 to 120 minutes of strength training per week was associated with lower long-term mortality, including mortality from neurological diseases.
Of course, this does not prove that strength training prevents dementia. It does, however, add to the broader evidence that maintaining muscle and physical function is important for healthy aging.
The Bigger Picture
The risk for dementia is affected by a number of factors, including age, genetics, cardiovascular health, diabetes, physical activity, sleep and lifestyle. Muscle health is not a magic shield against dementia.
But the science is pointing toward a broader definition of healthy aging. Do not focus only on losing weight. Focus on losing excess fat while protecting the muscle underneath.
Because when it comes to long-term health, the goal may not simply be a smaller number on the scale.
It may be a stronger body supporting a healthier brain.
